Background of the Study
Health promotion programs are vital for preventing disease and enhancing quality of life, particularly among the elderly who are at increased risk of chronic illnesses. In Jigawa State, disparities in access to health promotion initiatives between urban and rural areas have been noted, prompting the need for a comparative study of elderly patients’ perceptions. Urban areas often benefit from better healthcare infrastructure, more comprehensive programs, and higher literacy levels, which can influence the effectiveness of health promotion strategies (Umar & Ahmed, 2023). Conversely, rural areas may face challenges such as limited healthcare resources, lower health literacy, and cultural barriers that affect the uptake and success of these programs (Mustapha, 2024).

Elderly patients, due to their vulnerability to chronic conditions, rely heavily on health promotion programs that emphasize preventive care, regular screenings, and lifestyle modifications. However, the perception and acceptance of these programs can vary significantly based on geographic location and socio-economic status. In urban settings, elderly patients might have greater exposure to information through digital media and community outreach initiatives, which may enhance their participation and adherence to health promotion activities (Bukar, 2024). In contrast, rural elderly populations may depend more on traditional practices and community-based approaches, which could lead to a different interpretation of health promotion messages.

Recent research indicates that patient perception is a key determinant of the success of health promotion programs. Positive perceptions can lead to increased engagement, better adherence to preventive measures, and overall improved health outcomes (Yusuf et al., 2025). Despite this, there is a paucity of studies that directly compare the perceptions of elderly patients in urban versus rural contexts within Jigawa State. This study intends to bridge that gap by analyzing how various factors such as accessibility, cultural beliefs, and program delivery methods shape elderly patients’ attitudes toward health promotion.

Moreover, understanding these perceptions is critical for tailoring health promotion programs that are culturally sensitive and contextually appropriate. The findings could inform policymakers and healthcare providers on how to design and implement more effective interventions that cater to the unique needs of both urban and rural elderly populations. In doing so, the study contributes to the broader goal of reducing health disparities and enhancing the quality of life for the elderly in Jigawa State (Adamu & Suleiman, 2023).

Statement of the Problem
Despite the recognized importance of health promotion programs for elderly populations, there exists a significant disparity in how these programs are perceived by elderly patients in urban and rural areas of Jigawa State. Preliminary observations indicate that urban elderly individuals tend to have a more positive perception of health promotion initiatives compared to their rural counterparts. This difference may be attributed to variations in accessibility, program design, and communication channels between the two areas (Ibrahim, 2024). In urban settings, better access to healthcare facilities, more frequent community outreach, and the availability of digital information sources contribute to a higher level of awareness and acceptance of health promotion programs. Rural areas, on the other hand, often face infrastructural challenges, lower health literacy, and cultural factors that may hinder the effective dissemination and acceptance of these programs.

The problem is further compounded by the lack of comprehensive studies that compare these perceptions within the context of Jigawa State. Without a clear understanding of the factors influencing elderly patients’ perceptions, it is challenging for policymakers and healthcare providers to design interventions that are both effective and culturally appropriate. Moreover, the limited research on this topic restricts the ability to address the unique needs of rural populations, potentially widening the health disparities between urban and rural areas.

Additionally, the existing literature does not adequately explore how socioeconomic status, educational background, and previous experiences with the healthcare system influence elderly patients’ perceptions of health promotion programs. These factors are critical in shaping attitudes and determining the level of engagement in preventive health behaviors. The lack of comparative data leaves a significant gap in knowledge, undermining efforts to tailor health promotion programs that are responsive to the specific needs of diverse populations in Jigawa State (Sani et al., 2025).

Objectives of the Study

  1. To compare the perceptions of elderly patients on health promotion programs between urban and rural areas of Jigawa State.
  2. To identify the key factors that influence these perceptions, including accessibility, cultural beliefs, and socioeconomic status.
  3. To recommend strategies for designing culturally sensitive and contextually appropriate health promotion programs for the elderly.

Research Questions

  1. How do elderly patients’ perceptions of health promotion programs differ between urban and rural areas of Jigawa State?
  2. What are the key factors influencing these perceptions among the elderly in the two settings?
  3. What strategies can be implemented to enhance the effectiveness of health promotion programs for elderly patients in both urban and rural areas?

Research Hypotheses

  1. There is a statistically significant difference in the perception of health promotion programs between elderly patients in urban and rural areas of Jigawa State.
  2. Accessibility, cultural beliefs, and socioeconomic status significantly influence elderly patients’ perceptions of health promotion programs.
  3. Implementing tailored health promotion strategies will significantly improve the perception and engagement of elderly patients in both urban and rural areas.

Scope and Limitations of the Study
This study will focus on elderly patients in selected urban and rural communities within Jigawa State. Data will be collected through structured questionnaires and in-depth interviews. Limitations include potential response bias, the variability of program implementation across different communities, and challenges in generalizing the findings to all elderly populations in Jigawa State.
